Before your first visit, we recommend calling your insurance company to confirm your physical therapy benefits. Most insurance plans let you refer yourself to physical therapy, but some may need a referral from your doctor. If you’re unsure about coverage, call us at (206) 327-9880.

We accept most insurance plans!

If your insurance is out of network:

  • The cost is $180, and we can bill your insurance for you.
  • If you choose not to bill insurance, the cost is $165, and we’ll give you a superbill so you can send it to your insurance yourself.

As of October 31st, 2023, Pain Science Physical Therapy will no longer be in-network with United Health Care/Optum/AARP Med Advantage plans.

Co-pay

If you have a co-pay, please be ready to pay at each visit. For your first visit, we’ll check with your insurance, but it’s not a guarantee of payment. We encourage you to contact your insurance company, too.

We accept most major insurances.

Work Injury

For workman’s compensation claims, please have your claim number, adjuster’s name, and phone number ready when you call to make your first appointment. We’ll verify your claim and contact you to schedule your visit.

Car Accidents

If your treatment is due to a car accident, have your vehicle insurance information ready when you call. We’ll check that you have PIP (Personal Injury Protection) before scheduling.
